Incident Investigation

Identifying the human slips and lapses that, if remain unchecked, can lead to an accident or disaster

Incident Investigation

Despite effective risk assessment programmes, incidents still happen and thorough investigations are necessary to identify the root causes. Today’s high pressure commercial environments frequently and cruelly expose the human element such that relatively minor errors can have wholly disproportionate consequences. Energy spent examining and reducing the causes of those errors will have a direct bearing on the wellbeing of the workforce and on the effectiveness of the company.

WrightWay’s work in this area follows three core strands: conducting an investigation on behalf of a client, aiding a client to examine their own working environment and finally teaching the workforce to proactively examine and rectify potential risks and danger areas.

Incident investigation is a post-event activity occurring after any damage has been done and it is this investigative process which identifies areas of concern. Incident investigation requires the ability to examine the root causes of an accident rather than focus on the consequences; to examine the potential, not the outcome.

WrightWay introduces clients to the concepts and practicalities required to investigate incidents so they are able to uncover incident root causes within the organisation’s management and operating processes. This affords a company the opportunity to make meaningful changes to its practices, which in turn will reduce incidents and improve business effectiveness.
